Usually authentication. If SPF, DKIM and DMARC are not configured correctly for your sending domain, mailbox providers have no reason to trust you. After that it is list health - sending to stale addresses and spam traps destroys reputation fast. Content is a distant third, despite being where people look first.
Determined by relevance, not a schedule. A monthly newsletter nobody opens is worse than nothing, because low engagement damages deliverability for everything else you send. We would rather segment smaller and send more relevant, less frequent mail.
Usually, though it takes time. The process is clean the list, authenticate properly, then warm the domain back up by sending first to your most engaged contacts and widening gradually. Expect four to eight weeks. The alternative - moving to a new domain - just moves the problem.
